A Historical Perspective on Aviation Accident Investigation

چکیده
When the editor suggested that we write a piece for this 50 edition of the newsletter, it seemed appropriate to reflect on the longer term history of our field and, in particular, to focus on the topic of accident investigation which has been the focus of our previous work. In early 1928, the Assistant Secretaries for Aeronautics in the Departments of War, Navy, and Commerce in the United States, asked the National Advisory Committee for Aeronautics (N.A.C.A.) to develop a common approach for the analysis and reporting of aircraft accidents. In response to this request, the N.A.C.A. organized the Special Committee on the Nomenclature, Subdivision, and Classification of Aircraft Accidents. The following paragraphs review the work of this special committee and reflect on the changes that have taken place since this early initiative to learn from adverse events in aviation.

Human Operator Behavior Modeling in Real-World Environment
From the perspective of activity theory and object-oriented analysis and design our approach provides a comprehensive modeling of human operator’s performance. The modeling is based on theory of frames, modal logic, decision making procedures, and psychological experiments. It takes into account personal and social factors of operator activity, his conscious and unconscious motives, and also or...
متن کاملAccident modeling: From symptom to system
A series of surveys on accident investigation models show a wide variety of models, dedicated to specific industrial applications, domains and investigation aspects. In particular the investigation of human factors is exposed to a wide diversity of models. In reviewing such models, the majority proves to be a derivate from the Reason’s Swiss Cheese causation model or the Rasmussen model on syst...
متن کاملHuman Error and Accident Causation Theories, Frameworks and Analytical Techniques: An Annotated Bibliography
Over the last several decades, humans have played a progressively more important causal role in aviation accidents as aircraft have become more. Consequently, a growing number of aviation organizations are tasking their safety personnel with developing safety programs to address the highly complex and often nebulous issue of human error. However, there is generally no “off-the-shelf” or standar...
متن کاملEstimation of Air Traffic Accident Cost in Nigeria
This project proposes an accident cost estimation model in the aviation industry in Nigeria. The model takes into consideration the probability of accidents and the cost of the potential consequences. Potential consequences of accidents are classified as human severity, property damage, and economic implications. The three classes of accidents identified in this study were fatal, serious, and m...

Introduction: The resilience of an architectural space is the ability to deal with damages caused by an accident, which helps to return to a normal condition after the accident. One of the important aspects of resilience is the physical aspect, which means the ability of a building to withstand changes, events, and the passage of time, and the response of the building to current and future need...
